We are looking for a licensed Physical Therapist to join our team at a skilled nursing facility. As the Physical Therapist, your role will involve evaluating and treating residents with physical impairments and functional limitations to help them achieve their highest level of mobility and independence.

Job Description :

• Assess residents' physical abilities, functional limitations, and mobility needs using standardized assessments
• Create and execute personalized treatment plans based on assessment outcomes, resident goals, and physician directives
• Deliver skilled physical therapy treatments, including therapeutic exercises, manual therapy, and modalities
• Track and document resident progress, adjusting treatment plans as needed
• Collaborate with interdisciplinary team members such as physicians, nurses, occupational therapists, and speech therapists to ensure coordinated care and optimal resident outcomes
• Educate residents and family members on physical therapy objectives, treatment plans, and home exercise routines
• Maintain accurate and timely records of physical therapy services rendered
• Participate in team meetings, in-service training, and quality improvement projects
• Stay updated on physical therapy practices and relevant regulations
• Qualifications :
• Current California license to practice as a Physical Therapist
• At least 2 years of experience in a healthcare setting, preferably in a skilled nursing facility
• Familiarity with current physical therapy practice standards and regulations
