About The Position
- Job Title : Medical Social Worker Job Specialty : Licensed Clinical Social Worker (LCSW) Job Duration : 13 weeks Shift : Day 5x8-Hour (08 : 00 - 16 : 30), Monday to Friday Guaranteed Hours : Not specified Experience : Acute care experience required;
- minimum 1 year of experience accepted License : LA License required (takes 6-8 weeks to acquire) Certifications : LCSW (LMSW accepted);
Certifications such as ACM, CCM preferred; EPIC experience required Must-Have : - COVID-19 Vaccine : Required (Medical / Religious Exemptions Only)- Flu Vaccine : Required (Medical / Religious Exemptions Only)- Care Coordination skills- Discharge Planning expertise- Needs Assessment / Order DME capability Job Description : Provide short-term acute care and engage in inpatient medical social work- Coordinate services, home care, and equipment arrangements following patient discharge- Refer patients to rehabilitation and long-term care facilities as needed- Engage in case management and utilization review- Conduct needs assessments and plan of care- Float within the facility as required- Participate in a one-day orientation session (8 hours) Additional Notes : References from 2 Managers / Supervisors required to start- CPI certification necessary for CNA / ER nurses before startingCandidates should have a LA license upon submission and be prepared with the necessary certifications as outlined.
The role is not open to first-time travelers and requires applicants to live over 65 miles away from the facility.
