What you will Do : + Analyzes and differentiates clinical data to appropriately diagnose nursing problems; and identify nursing diagnosis.
- Provides individualized care to clients with complex care needs.+ Demonstrates the ability to utilize critical thinking skills in the home environment and identify situations requiring physician notification or emergent care.
- Demonstrates the ability to care for clients with a variety of diagnosis' including but not limited to orthopedic, surgical, medical, cardiac, and wound / ostomy.
- Provides health education to patients / families and populations focused on achieving lower costs, engaged patients and reduced hospitalizations and ER / Urgent visit usage.
- Delivers nursing care for the clients in their homes, utilizing appropriate available resources. Hours / Schedule : Monday through Friday 8-4 : 30.
Flexible schedule. Minimum Qualifications : + Current state LPN license.+ Must provide proof of Basic Life Support ( BLS Provider -American Heart Association or Professional Rescuer-American Red Cross) prior to employment or within 30 days of hire and maintains current status.
- Must provide proof of Mandatory Reporting for Child and Dependent Adult Abuse training prior to employment or within 6 months of hire and maintain current status per State of Iowa guidelines+ Motor Vehicle : Personal Vehicle.
