Position Title : Case Manager

Reporting Relationship : Lead Case Manager

Position Summary

The Case Manager for NYAP’s La Jornada (The Journey) Program focusing on Long-Term Foster Care (LTFC) works to make a meaningful difference in the lives of the minors who have fled their country of origin and arrived in the without a parent or legal guardian.

The Case Manager works closely with minors, foster families, sponsor families, and community partners to provide advocacy and support towards targeted goals.

RESPONSIBILITIES

The Case Manager will perform duties including, but not limited to :

Performing all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies.

Responsible for following the assigned intake, placement, and discharge procedures including conducting / coordinating / reviewing / updating assigned assessments, service plans, forms, and packets and collecting and uploading assigned documents into NYAP’s electronic health record (EHR) system and the government’s database (UCP) within the mandatory timeframe.

Writing required and additional progress notes for assigned responsibilities, and uploading progress notes and accompanying documents to NYAP’s electronic health record (EHR) system and the government’s database (UCP) within the mandatory timeframe.

Coordinating with the team to identify minors’ primary sponsor, conducting and uploading sponsor assessments, and writing and uploading progress notes to the EHR and UCP within required timeframe.

Conducting case reviews, including sponsor status update, and uploading documents and progress notes to EHR and UCP within required timeframe.

Traveling to foster family and sponsor homes to conduct home visits and completing and revising assessments with all family members that live in the home to assess the family’s strengths and needs, as well as risk of harm to minors in the home.

Preparing for and participating in administrative and legal reviews of the case plan.

Maintaining contact with families through regular and planned, as well as unannounced, visitation of parents and minors.

Communicating with potential foster families and sponsors, and gathering necessary information and documentation.

Talking to relevant stakeholders and assessing foster families and sponsors to formulate recommendations to Lead Case Manager.

Performing duties on-call as outlined in on-call description when called upon to respond to after-hours emergencies.

Attending weekly supervision meetings and actively participating in the supervision process and participating in meetings and conferences with agency personnel and community providers.

Making immediate reports to the supervisor of incidents involving risk of harm to minors or matters of concern affecting agency function.

Keeping family case record documentation properly organized and filed according to HIPPA, COA, and agency policies.

Performing other duties as requested.

MINIMUM QUALIFICATIONS

Bachelor degree in education, psychology, sociology or other relevant behavioral science field.

Experience in the delivery of social services in a diverse cultural population with minors including infants, children, and adolescents.

Bilingual English / Spanish fluency required.

Must be able to organize, navigate, and physically escort youth to various travel destinations possibly including airlines, trains, and buses.

Travel includes out-of-state trips unify youth with families.

A willingness to work flexible and non-traditional hours in the service of foster caregivers, families of origin, and persons served.

Capacity to remain objective, professional, and effectively work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds.

Possess a sincere desire and ability to advocate for children facing behavioral health, educational, legal, development, and socio-economic challenges.

Works well independently and as a team member.

Demonstrate exceptional customer service focused on caring for people, connecting communities, and promoting peace.

  • Must have a valid driver’s license, reliable transportation, automotive insurance, and a good driving record.
  • Minimum automobile insurance coverage of $100,000 / 300,000 bodily injury liability.

Proficient use of desktop and laptop computers, internet search, people management programs, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.
