The Allegheny County Department of Human Services is seeking a dedicated and dynamic Program Manager to join the Behavioral Health Technology team and manage the MATTERS Platform, a client management and referral application designed to connect individuals presenting in the emergency room with substance use disorder (SUD) to essential services.
This role encompasses programmatic responsibilities, including onboarding new providers, customer service, and connections to care.
Primary Responsibilities Provider Onboarding and Training Build and maintain relationships with emergency departments, correctional health facilities, urgent cares, inpatient units, and community-based organizations.
Coordinate and train these referral partners to ensure they can effectively use the MATTERS platform to link individuals to community services.
Facilitate regular stakeholder meetings and provide ongoing support and training to ensure continued access to care. Customer Service and Support Serve as the primary point of contact for providers and users within the MATTERS network.
Provide technical assistance and troubleshoot issues related to connecting individuals to care through the MATTERS platform.
Ensure timely follow-up and support for individuals referred through the MATTERS platform. Technology and Feature Management Collaborate with the development team to ensure the platform meets the evolving needs of users and stakeholders.
Program Expansion and Integration Lead efforts to expand the integration of the MATTERS platform across various agencies, including hospitals, treatment organizations, EMS agencies, correctional facilities, peer organizations, transportation providers, and pharmacies.
Coordinate with local treatment organizations to maintain updated appointment availability and contacts. Reporting and Documentation Maintain accurate and up-to-date records of all patient-related encounters and follow-ups.
Document all interactions and activities in compliance with MATTERS standards. Prepare and submit regular reports on MATTERS-related activities and outcomes.
