Emergency Department Case Manager (RN) needed for a night-shift contract opportunity with Yoh’s client located in Palo Alto, CA!

Top Skills Should You Possess :

• Acute Care or Emergency Department Nursing Experience
• Recent Case Management Experience
• Ability to work M-F, 2 : 30 PM-1 : 00 AM with rotating weekends (Sunday-Saturday)

What You’ll Be Doing :

• Responsible for coordinating continuum of care and discharge planning activities for a caseload of assigned patients.
• Coordinating all facets of a patient's admission / discharge.
• Performing utilization review activities, including review of patient charts for timeliness of services as well as appropriate utilization of services.
• Ensuring optimum use of resources, service delivery, and compliance with external review agencies' requirements.
• Act as consultants to the clinical team, service lines, and other departments regarding patient assessment and patient care and participate in program development and quality improvement initiatives.
• Apply guidelines and collaborating with multidisciplinary teams, influence and direct the delivery and quality of patient care.

What You Need to Bring to the Table :

• Active and unrestricted California RN license
• Bachelor’s Degree in Nursing preferred
• Current BLS certification
• Two (2) years of progressively responsible and directly-related work experience
• Previous Case Management experience, preferably in an Acute Care and Emergency Department Nursing Experience setting
• Excellent verbal and written communication skills
• Excellent computer skills
