- Summary Reporting to the Director of Advanced Practice Providers, the Nurse Practitioner (NP) will plan, direct, and evaluate health care in home health and hospital at home health services;
- examine and treat chronic and acute episodic illnesses, minor injuries, accidents, and other injuries / illnesses; performs selected laboratory testing;
offer referrals to physicians, write prescriptions, and perform home procedures within the scope of expertise, protocols, and available equipment.
The NP will also work with the medical director in the care and treatment of home health and hospital at home patients. Essential Job Functions 1.
Meets with new patients to orient them about services, to identify needs, and to suggest physicians and services. 2. Sees hospital at home and other in-home patients for laboratory testing, examination, diagnosis, and treatment of chronic and acute episodic illnesses and minor injuries.
Refers to physician as needed. Writes prescriptions and performs office procedures within the scope of expertise, protocols, and available equipment.
Makes house calls when necessary. Schedule’s follow-up visits, including telehealth, as needed. 3. Performs physical examinations.
4. Acts as a resource person for decisions, planning, and policies affecting the health of patients. 5. Maintains patient records.
Records statistics and conducts research as deemed necessary by the Company. Compiles reports concerning care at patient’s home.
6. Collaborates with staff to identify patients' needs and to help develop plan to meet these needs. 7. Follows patients in hospital at home as needed for continuity of care.
8. Other duties as assigned. Minimum Qualifications 1. Master’s Degree in Science of Nursing or Physician Assistant Studies or related required 2.
Minimum 3 years of advanced practice provider experience required, including experience in Home Healthcare. 3. Advanced Practice Registered Nurse License or Physician Assistant License in the State of Texas required 4.
Family Nurse Practitioner or Adult Gerontology or Acute Care Nurse Practitioner certification or Physician Assistant Certification from National Commission on Certification of Physician Assistants required 5.
American Red Cross or American Heart Association Basic Life Support Course (BLS or BLCLS) and Certification 6. American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification 7.
Satisfactory references from employers and / or professional peers 8. Valid Driver's License and ability to travel throughout the area Powered by JazzHR
