Schedule : every fourth weekend, work Mon. - Sun. and
call all days
Call : 24 hr. call Mon. - Sun.
Dated Needed : 10 / 11 - 10 / 18
10 / 11 - 10 / 18
10 / 25 - 11 / 1
11 / 25 - 11 / 30
12 / 23 - 12 / 30
Mostly acute care, take call when working.
Trauma Level 3 Facility
EMR : eCW Clinical Works OP; Quadramed IP
Expenses covered, weekly pay and Occurrence Form
