Position : IDD / Behavioral Nurse (LPN / RN) Location : philadelphia, PA
Job Id : 338 # of Openings : 10 The position is responsible for overall medical needs of assigned individuals in our program.
Performs nursing duties including assessment, treatment and follow through resolution of all health issues. The Nurse (RN / LPN) ensures the effective execution of the nursing process by creating, implementing, and reviewing nursing procedures, and providing direct supervision of participant’s health evaluations and processes;
ensures that all programs are adequately supported for all medical needs and requirements as per 6400 Regulations. Oversees all nursing services delivery to include labor, supplies, pharmaceuticals, durable medical equipment, and billing. Job Duties
- Provide medication monitoring and education to individuals as determined in the treatment plan.
- Coordinate all medical care for individuals with all medical providers, dental, vision, etc.
- Develop comprehensive goal plans in conjunction with individuals and family members.
- Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.
- Educate and support individual's families and advocate for individual's rights and preferences.
- Provide on call crisis intervention as required.
- Participate in daily organizational staff meetings and treatment plan meetings.
Requirements
- Must be trained on ISPs of all individuals they support.
- Completion of agency’s 24 hours of annual training
- Must be a graduate of an accredited school of nursing and have a license to practice nursing as a Registered Nurse in the Commonwealth of Pennsylvania.
LPN or BSN required; MSN preferred.
- At least three (3) years supervisory experience required, preferably in an IDD / behavioral health care setting.
- Requires demonstrated leadership abilities. Excellent oral and written communication skills.
- Proficient computer skills and mastery of Microsoft applications. Must possess and maintain a valid driver's license and be at least 21 years of age.
