The Community Health Worker / Peer Support Specialist for our Behavioral Health Home program provides community-based integrated physical and behavioral healthcare services to individuals who may have complex and chronic mental illness, substance use, and medical co-morbidity in a fast-paced, interdisciplinary team setting.

This position will work with individuals served to enhance access to services and work to achieve goals for mental and physical wellness.

This is primarily a community-based position. Staff will have to opportunity to work remotely on occasion and will also be expected to be in the office at times as needed.

Qualifications : Education

Education

  • Required :
  • High school diploma or equivalent

Experience

  • Required :
  • Office management experience
  • State-certified peer support specialist or trained Community Health Worker
  • Preferred :
  • Experience working with persons who are mentally ill in either a hospital or community setting

Licenses & Certifications

  • Valid and unrestricted drivers license
