We areseeking a talented Hospice Registered Nurse (RN) Case Managerfor 40 hours perweek .
JobSummary :
The Hospice RegisteredNurse (RN) Case Manager plans organizes and directs hospice careand is experienced in nursing with emphasis on community healtheducation and experience.
The professional nurse builds from theresources of the community to plan and direct services to meet theneeds of individuals and families within their homes andcommunities.
Responsibilities :
Completesan initial comprehensive and ongoing comprehensive assessment ofpatient and family to determine hospice needs.
Provides a completephysical assessment and history of current and previousillness.
- Provides professional nursing care byutilizing all elements of nursingprocess.
- Assesses and evaluates patients statusby :
- Writing and initiating plan ofcare
- Regularly reevaluating patient andfamily / caregiver needs
- Participating inrevising the plan of care asnecessary
- Initiates the plan of care and makesnecessary revisions as patient status and needschange.
- Uses health assessment data todetermine nursing diagnosis.
- Develops a careplan that establishes goals based on nursing diagnosis andincorporates palliative nursing actions. Includes the patient andthe family in the planning process.
- Initiatesappropriate preventive and rehabilitative nursing procedures.Administers medications and treatments as prescribed by thephysician in the physicians plan ofcare.
- Counsels the patient and family inmeeting nursing and related needs.
- Provideshealth care instructions to the patient as appropriate perassessment and plan.
- Assists the patient withthe activities of daily living and facilitates the patients effortstoward selfsufficiency and optional comfortcare.
- Acts as RN Case Manager when assigned byClinical Supervisor and assumes responsibility to coordinatepatient care for assignedcaseload.
- Participates in oncall duties asdefined by the oncall policy.
- Ensures thatarrangements for equipment and other necessary items and servicesare available.
- Supervises ancillary personneland delegates responsibilities whenrequired.
- Assumes responsibility for personalgrowth and development and maintains and upgrades professionalknowledge and practice skills through attendance and participationin continuing education and inserviceclasses.
- Fulfills the obligation of requestedand / or accepted case assignments.
- Activelyparticipates in quality assessment performance improvement teamsandactivities.
Communication :
- Completesmaintains and submits accurate and relevant clinical notesregarding patients condition and care given. Records pain / symptommanagement changes / outcomes asappropriate.
- Communicates with the physicianregarding the patients needs and reports changes in the patientscondition; obtains / receives physicians orders asrequired.
- Communicates with community healthrelated persons to coordinate the careplan.
- Teaches the patient and family / caregiverselfcare techniques as appropriate. Provides medication diet andother instructions as ordered by the physician and recognizes andutilizes opportunities for health counseling with patients andfamilies / caregivers.
Works in concert with the interdisciplinarygroup.
- Provides and maintains a safeenvironment for the patient.
- Assists thepatient and family / caregiver and other team members in providingcontinuity of care.
- Works in cooperation withthe family / caregiver and hospice interdisciplinary group to meetthe emotional needs of the patient andfamily / caregiver.
- Attends interdisciplinarygroupmeetings.
Qualifications :
- Apassion for hospice care and the ability to build relationshipswith both patients and their familymembers.
- The ability to work efficiently withlittle direct supervision.
- A valid driverslicense and an insured automobile that is in good workingorder.
- 12 years of recent acute care experiencein an institutional setting (required).12 years of recent home careexperience (preferred).
Were excited totrain RNs that are passionate about hospicecare.
- Graduation froman accredited school of nursing.
- Understandshospice philosophy secure with issues of death / dying. Comfortableproviding specialized care to the terminallyill.
- Displays empathy for the needs of the illinjured frail and impaired.
- Provides a calmdemeanor when in a patientshome.

Schedule :
- MondayFriday; 8a5p with some oncall and weekendrotation.

Compensation :
- $75000$90000 per / year
- Mileage reimbursement at $0.58per / mile
