• Shift : 3 : 00 pm - 11 : 00 pm, with an every other weekend rotation

A little more about our center :

Southview Manor (formerly Southview Care) is an inner-city skilled nursing facility (SNF) with 5 floors and an average census of 160+.

We have and multiple levels of care, including long term care and a large behavioral health (formerly indigent) population.

• 5 star Quality Measures this means we provide excellent clinical care!
• The average age of our patients is 35-55 and the most common diagnoses are schizophrenia, bi-polar and major depressive disorders.
• Many are able to age in place, and about 25% return to the community.
• We have several programs encouraging engagement, but the most popular spot in our center is the game room.
• We love new grads and work with students from multiple schools / universities in the area.
• The Metra and the Green Line are just a few blocks away

The National Association of Rehab Providers & Agencies ( NARA ) awarded Transitional Care Management their Impact Award for 2023 for our Life Skills Programming.

Job Description

What you will do in this role :

As a Registered Nurse (RN) or Licensed Practical Nurse (LPN), you will be joining a team of nursing professionals dedicated to providing excellent clinical care.

Some have been with us for a long time and can mentor others. You will help patients reach their clinical goals and work side by side with a highly skilled multidisciplinary team.

Qualifications

What we need from you :

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in IL
• Prior floor nurse experience within a skilled nursing facility is helpful
• New Graduate Nurses are welcome to apply!
• Electronic medical records experience is helpful (Point Click Care or PCC is ideal)
