Peak Health provides wellness programs for various organizations (clients). Nurses will help individuals to identify their risk factors, assist with establishing individual goals and motivate / coach participants to change lifestyle behaviors in order to change the risk factors.

ESSENTIAL DUTIES Following is a summary of the essential functions for this job. Other duties may be performed, both major and minor, which are not mentioned below.

Specific activities may change from time to time.1. Registered nurse travels to assigned client employment site to provide health evaluations for the participants, including : a.

Reviewing the individually completed Health History Questionnaireb. Reviewing and interpreting individual blood lab test resultsc.

Obtaining individual weight, blood pressure, pulse, body fat or body mass, and fitness level.d. Establishing individual goals by making recommendations for improving the individual's health risk.

2. May conduct evaluation via teleconference when no site is available.3. May perform other tasks as assigned by Nurse Manager.

QUALIFICATIONSRequired Qualifications : The requirements listed below are representative of the knowledge, skill and / or ability required.

Starting Salary Range : $47,500
