About The Position
Job Title : Physical Therapy Assistant Job Specialty : Rehabilitation Therapy Job Duration : 13 weeks Shift : Monday to Friday Guaranteed Hours : 32 per week Experience : New graduates are welcome;
Skilled Nursing Facility (SNF) experience is preferred. License : State License required Certifications : None specified Must-Have : Resume and license verification- Prescreen document completed- Criminal background checks (7 years for county, federal, and multi-state)- Nationwide Sex Offender Search (7 years or more)- HHS / OIG Search and GSA Search- Social Security Number Trace- 2 Step PPD, Quantiferon, or Proof of Positive PPD and Chest X-Ray- SAM Search- Oregon State Background Check Job Description : Ensure prescreen is completed and attached during candidate submission.
- Attach resume and license verification; candidates without these will be declined.- Conduct a COVID test 72 hours prior to the start date with results ready 48 hours before starting.
- Understand and fulfill all credentialing requirements listed.
