Job Description

The Physical Therapist (PT) evaluates and treats patient’s mobility and motor assets and liabilities for functional performance.

Duties / Responsibilities :

• Provides goal directed, activity oriented treatment to promote optimum independence in functional mobility and daily life tasks.
• Ensures continuation of performance through effective education of the patient, family or support system.
• Provides adaptive methods and equipment as appropriate to enhance performance
• Conduct effective and appropriate evaluation and treatment interventions and communicate with, supervise and utilize assistive personnel appropriately.
• Participate in the ongoing review and evaluation of Quality / Performance Improvement program.
• Prepare and submit all required reports in a timely manner.
• Maintain timely filing and completing of required documentation.
• Perform and conduct training as requested.
• Participate in meetings and committees as designated to promote the development of the area and / or clinical specialty.
• Work with other therapists to maintain level of care and communication of assigned patients.
• Assume responsibility for equipment, inventory, safety, and security of assigned facilities.
• Maintain compliance with external agency requirements as designed.
• Perform in a manner, which strengthens the coordination and cooperation of all organizational components of Valir Health.
• Any and all other duties or responsibilities assigned.

Education, Licenses, Certifications and Experience :

• Licensed PT or licensure eligible in the State of Oklahoma
• Current CPR certification
• Bachelor’s degree
• Master’s degree preferred
• Experience in Rehab preferred

Physical Requirements :

• Ability to speak, hear, and read English
• Ability to discern temperature, texture, size, and shape
• Lift up to 75# from floor to 19 and 100# 19 to 25 (Occasional)
• Front Carry two handed carry of 25# up to 20 feet (Occasional)
• Grasp of various sizes and weights up to 50# (Constant)

• Walk up to .25 mile (Frequent)
• Sit (Occasional)
• Sustained Forward bending in standing (Occasional)
• Sustained and repetitive squat (Occasional)

• Balance (Continuous) in walking, standing, carrying, moving patients, and equipment, etc.

Safe Environment :

• Demonstrate knowledge of Hospital specific and individual role in the fire / safety program including the following management plans : safety, security, hazardous materials / waste, emergency preparedness, life safety, medical equipment and utility systems.
• Participate in the Hospital policies and procedures relating to infection control for self, patients, and staff.
• Observe proper body mechanics and safety precautions in all job duties.
• Communicate information about incidents involving self, patients, staff and / or visitors to appropriate parties, using facility incident reporting systems as appropriate.
• Demonstrates the safe operation of equipment and machinery and follows procedures for reporting and correcting an unsafe situation.
