DescriptionThe Lead Community Health Worker serves as a role model to the team by promoting a positive and professional demeanor towards neighbors, staff members and team.

The Lead CHW will support daily operations of the CHW team as well as support a small case load of neighbors and clients.

The CHW lead will support the program manager in ensuring high quality service for all neighbors and patients served by the CHW program.
