Responsibilities
- Direct function and personnel of the care management department
- Develops, manages, and oversees the budget
- Prepares and evaluates monthly, quarterly, and annual reports of the department's functions
- Monitors and reports on changes in regulations and documentation issues to physicians and others as needed
- Performs other job-related duties as assigned
Qualifications
- Bachelor's Degree or equivalent combination of education
- Master's Degree, preferred
- 7 years hospital discharge planning, case management, and social work experience, preferred
- 5 years leadership experience, preferred
- RN and LCSW required
- LMSW, preferred
- MD (medical license) required
