Requirements :
- Must have DC License
- Must have experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters
- Must have 1 prior travel assignment
- LMSW / LGSW / LCSW
- Home Health, Hospice, telephonic, previous charge experience preferred
Must have DC License in hand
Shifts : 8am 4 : 30pm Day and rotating weekends
START DATE : 10 / 14
Will accept RN Discharge Planner or Licensed Social Worker
Social worker must be eligible for LMSW / LGSW / LCSW - they must have taken and passed the exam already
Certs Required : BLS, RN or LMSW / LGSW / LCSW
Work Setting skills Required : Cerner, Acute Hospital, Insurance Company, Long Term Acute Care / Rehab / Skilled Nursing
Work Setting skills Preferred : Home Health, Hospice, telephonic, previous charge experience
Case Management / Utilization Review skills required : Admission Criteria, Benefits Eligibility, Care coordination, Discharge Planning, Needs Assessment / Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, Worker’s comp case management
Regulatory Skills Required : CMS : Centers for Medicare and Medicaid Services, Department of Health, HIPAA (The Health Insurance Portability and Accountability Act), The Joint Commission / Core Measure / National Safety Goals
Regulatory Skills Preferred : Worker’s Compensation
Any Special Requirements : Must have experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters
Patient Diagnosis : Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU
Cerner is required
Uniform : Business Casual - Associate’s appearance should be clean, neat and professional. NO SCRUBS
2 Years Minimum Experience Required
