About The Position
Job Title : Social Worker - LMSW Care Management Job Specialty : Acute Care Management - LMSW Job Duration : 13 Weeks Shift : 5x8.
0 Guaranteed Hours : To be determined Experience : Two years of related experience in Health Care; Counseling and discharge planning experience.
License : Licensed Master Social Work (LMSW) or Licensed Clinical Social Worker (LCSW) in NY State or eligible for NY State license.
Certifications : Screen certification or experience or intent to pursue within six months. Must-Have : Master's degree in Social Work from an accredited program.
- Ability to handle confidential information with discretion.- Professional and courteous demeanor.- Ability to meet deadlines and manage multiple priorities.
- Strong interpersonal, communication, and organizational skills.- Computer literacy. Job Description : Identify patient and family psychosocial and environmental needs related to admission, diagnosis, treatment, and discharge.
- Provide evidence-based care while promoting cost-effective utilization of hospital resources.- Collaborate with an interdisciplinary team to develop and implement a comprehensive discharge plan for positive patient and family outcomes.
- Make appropriate referrals for community services and provide emotional support, therapeutic interventions, crisis intervention, bereavement services, and patient / family education as needed.
- Assess and intervene in situations requiring mandatory reporting.- Provide supervision to graduate social work student interns.
- Identify and resolve barriers for patients presenting with non-emergent needs, educating them on accessing primary and preventive services.
- Educate patients on health care services roles, including PCP, Urgent Care, and ED, and refer to Health Home Care Manager if necessary.
- Educate patients on transportation resources within the community. Preferred Qualifications : Experience in an Acute Hospital setting and / or Nursing Home knowledge.
- Prior experience conducting Crisis Intervention Evaluations.- Experience with Microsoft Office products and electronic medical records.
