We strive to provide QUALITY services to our patients.Reporting to the Coordinator Of Care (COC), the Field RN assesses, plans and coordinates home care services for assigned patient caseload and provides skilled nursing care in accordance with professional standards, regulatory guidelines, meeting agency standards, and obtaining appropriate certification.

Duties and ResponsibilitiesPerform patient assessments to implement the best plan of careCoordinate resources & services for assigned caseload;

working with the patient, family & physicianMaintain up-to-date / accurate documentation for assigned casesMaintains a cooperative relationship among health care teamsJob Qualifications RequiredActive and unrestricted NYS RN licenseExcellent written and verbal communication skillsAbility to meet tight deadlines.
