Description
- Arranges for thepost-discharge needs of patients.
- Collaborates withproviders, nurses, patients, families, and other care providers inthe development and implementation of Quality programs andinitiatives and patient care plans, including discharge planning,across the continuum of care in support of the GCRMC Mission.
- Communicates withproviders regarding patient status and assist him / her in ensuringproper placement of patients.
- Demonstratesfamiliarity with the various levels of care and community resourcesavailable for each patient and the providers of these differentaspects of care.
- Utilizesinnovative methods to facilitate development and implementation ofdischarge plans.
- Attends CommunityHealth Workers Initiative and New Mexico Department of Healthmeetings as directed.
- Perform advocacyactivities to support vaccine equity.
- Provides COVID-19related outreach and education to underserved and racial / ethniccommunities to promote vaccine healthliteracy.
- Conducts outreachactivities which may include community-based activities such asdoor-to-door outreach, attendance at community events, andassistance with the planning and coordination of vaccinationclinics.
- Promotes theimportance of self-care and other preventive healthmeasures.
Prepares andturns in monthly activity reports, compiles and formats datanecessary for departmental reports.
- Demonstrateknowledge in a medical office / medicalterminology.
- Facilitatephysician and patient communication.
- Answer incomingcalls in a professional and courteous manner with minimum rings.
- Route phone callsand handle all enquires efficiently and with courtesy.
- Welcome patientsand visitors in a polite and courteous manner.
- File and faxdocuments appropriately.
- Contribute toteam effort by accomplishing related tasks asneeded.
Participatesin quality improvement, continuing education, and evaluationactivities and other duties as deemednecessary.
