Job Description

The Psychiatric Partial Hospitalization Program is seeking a full-time Registered Nurse

Schedule : Monday through Friday, 8 : 00am to 4 : 30pm

Our partial hospitalization program fills a gap in mental health services by providing supportive and structured crisis intervention for individuals requiring more intensive mental health care than a weekly counseling or psychiatric appointment can provide.

It may also be utilized as a step down from inpatient psychiatric treatment. The Psychiatric PHP can see up to 24 patients per day.

The Registered Nurse in Psych PHP :

  • Works as part of an interprofessional team made up of Psychiatrists, counselors / therapists, to provide safe and effective care for patients diagnosed with behavioral health disorders
  • Admits new patients
  • Facilitates an orientation group for patients
  • Facilitates two psycho-education groups with documentation per day
  • Serves as a liaison between physician and patient for medical questions and concerns
  • Acts as a resource person for all things medical for interprofessional team
  • Obtains prior authorizations for medications and treatment days

The Registered Nurse provides knowledgeable and caring clinical practice and care coordination through an understanding of patients, families, nurses and healthcare delivery teams.

Consistently provides safe and therapeutic care in a holistic and systematic way while incorporating differences into the provision of care and conducting all patient care in a patient / family centered manner.

Integrates knowledge, skills and experiences to meet the needs of patients and families throughout the continuum, to include patient and family education.

Communicates effectively and works cooperatively with others while having respect for and an understanding of other clinical disciplines.

Determines processes for delivery of care from direct patient / family communication, technology and other healthcare team member collaboration.

Job Responsibilities

  • Performs comprehensive patient health assessments by interpreting multiple data sets and synthesizing their interrelationships
  • Formulates individualized plans of care based on assessment findings and interpreting trends in patient populations to achieve best practice
  • Conducts all patient care in a patient and family centered manner, integrating knowledge, skills and experiences to meet the needs of patients and families throughout the continuum of care
  • Advocates for patients' rights from the patient and family perspective and provides counsel while acting as a resource to resolve issues
  • Accesses resources from all disciplines to achieve desired patient outcomes and coordinates healthcare and alternate services for patient / family transition planning
  • Documents all patient care activities, as per documentation standards, and assists healthcare team members with documentation
  • Evaluates the process to achieve outcomes, anticipates patient variances and makes revisions to plans of care
  • Makes clinical decisions based on experience, patient population data and intuition while promoting the development of clinical decision making of all team members
  • Communicates effectively, works cooperatively with others, and has respect for and an understanding of other clinical disciplines
  • Assists staff to assess patients' learning needs and outcomes measurement of patient education
  • Provides direction and guidance to others regarding practice while serving as a clinical resource, preceptor, and mentor and delegates patient care activities and coordinates unit activities as needed
  • Utilizes standards, guidelines and pathways for care delivery and ensures that the process for delivery of care is synthesized from direct patient / family communication, technology and other healthcare team member collaboration
  • Demonstrates evidence of self-development by obtaining a minimum of six formal contact hours and maintains own Team Member Continuing Education Record
  • Evaluates clinical and quality outcomes and contributes to performance improvement initiatives
  • May perform additional duties as assigned

Requirements :

  • Experience Required : 1 year of Registered Nurse clinical experience. Psychiatric experience is preferred.
  • Education : BSN from an accredited school of nursing. If RN has an Associates Degree (ADN); must complete BSN within 5 years of start date.
  • Certification : BLS certification required through American Heart Association
  • Licensure - Licensed Registered Nurse or eligible for licensure in the Commonwealth of Virginia as a Registered Nurse
  • Special Skills - Proficient in English. Strong verbal and written communication skills. Exceptional interpersonal skills and the ability to display a caring demeanor toward others.

Proficient computer skills required.
