About The Position
Job Title : Level 2 CHHA Case Manager Registered Nurse (RN) Job Specialty : Case Manager RN Job Duration : 13 weeks Shift : Monday to Friday, 8-hour shifts, Day shift (8 : 00 am - 4 : 30 pm), with weekends as needed Guaranteed Hours : Not specified Experience : One year of experience as a clinical nurse in a hospital required, and 9 months of experience in home care preferred License : RN License (New York License ONLY) Certifications : BLS (any provider accepted) Must-Have : Certified Home Health Care experience- EPIC or Netsmart experience (preferred)- OASIS experience Job Description : Develop and revise treatment and discharge plans in collaboration with patients, families, physicians, care delivery teams, and third-party payers.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition within defined time frames.
- Process orders and notify physicians of patient needs and changes in condition. Complete certification / recertification orders and discharge summaries.
- Provide an environment promoting respect for patients, their privacy, confidentiality, and property.- Initiate appropriate preventive and rehabilitative strategies.
- Deliver services requiring substantial specialized nursing skills.- Consult with and educate patients, families, and team members regarding disease processes, self-care techniques, and preventive strategies.
- Supervise Licensed Practical Nurses (LPN) to ensure patient health, safety, and compliance with care plans.- Direct, coordinate, evaluate, and supervise the quality of patient care services provided by the agency.
- Interact with patients, physicians, referral sources, and others to maintain positive relationships.- Support Care Management philosophy by facilitating collaborative, customer-focused planning, and case management.
- Ensure completion of all necessary clinical records for compliance with state and federal legislation and timely service delivery.
- Maintain knowledge of all available community-based services.- Demonstrate flexibility and willingness to visit patients based on need.
- Resolve problems at the point of contact.- Obtain necessary patient authorizations for information release.- Attend patient care conferences for assigned cases.
- Monitor results, progress, and take corrective actions in response to quality indicators.- Pursue efforts to reduce avoidable costs and errors.
- Ensure visit authorizations and documentation reflect medical necessity for home-based care.- Promote a work environment of growth, learning, trust, and mutual respect.
- Participate in education and orientation programs, as needed.- Demonstrate commitment to professional growth and competency.
- Responsible for achieving established Key Performance Indicators (KPIs).- Perform other duties as requested.Ensure adherence to all guidelines and policies for patient care and safety, while providing high-quality service within the home health care setting.
