Polaris is a beautiful 100-bed skilled nursing facility located at Milford Wellness Village in Milford, DE. We are seeking highly skilled, enthusiastic, & passionate Registered Nurse (RN) to join our compassionate community and provide exceptional patient care
Polaris Healthcare & Rehabilitation Center offers a full range of on-site therapy services including subacute rehab, wound care, respite care, and skilled nursing in a homelike environment, managed under the expert direction of our medical team.
Polaris offers a continuum of the finest post-hospital recovery and extended-stay care. In our new, innovative residential and rehabilitative facility, we provide a safe and functional real-world environment for physical, occupational and speech therapy to restore the highest level of function.

Key Responsibilities :
- Assigning nursing staff
- Establish a compassionate environment by providing support to Residents and their families.
- Provide information to Residents and Staff by answering questions and requests.
- Maintain safe and clean working environment by following procedures, rules, and regulations.
- Maintain Resident confidence by monitoring confidential information processing.
- Maintain documentation of Resident care services by auditing Resident and department records.
- Maintain nursing supplies inventory and other responsibilities as required.
- Regular reports made to the ADON, DON
Qualifications :
- Experience in this field and possess quality skills to motivate and mentor other nursing staff to provide quality care to our residents.
- Possess active Delaware RN License, in good standing.
- Must have experience working with residents, in a Short / Long Term atmosphere.
- Flexible & open to helping others.
- Be able to successfully pass all pre-employment background & drug tests.
