A company is looking for an LPN Case Manager to perform clinical operations and medical management activities across the continuum of care.
Key ResponsibilitiesPerform case management and coordination of care within an interdisciplinary teamProvide health education, coaching, and treatment decision support for membersAssess, plan, implement, coordinate, monitor, and evaluate patient care activitiesRequired QualificationsCurrent unrestricted NJ LVN license2+ years of experience in a physician's office, clinical, or hospital settingKnowledge of chronic diseases, especially COPD / asthma, diabetes, CHF, and IHDProficient computer skills to work efficiently with electronic medical records
