As a Field Nurse Supervisor for our Licensed Home Care Service agency, you will play a crucial role in ensuring the delivery of high-quality home care services in compliance with the regulations set by the New York State Department of Health.

This per diem position combines clinical expertise with managerial responsibilities, overseeing various aspects of patient care, caregiver competency, and documentation.

This role is vital to the continued growth of Synergy HomeCare and will help shape how we interrelate with our Caregivers and other senior-focused agencies.

Key Responsibilities :

  • Ensures the quality of patient care and service by using specialized knowledge and judgment to ensure that professional standards are in compliance with applicable State Practice Acts and Agency policies.
  • Coordinates with home office staff to ensure the appropriate assignment of personnel to meet the specific needs of an individual client.
  • Builds and maintains relationships with clients, community agencies, and professional organizations.
  • Maintains compliance with all agency clinical and administrative policies, procedures, and regulatory requirements.
  • Knows and interprets community resources available for continuity of patient care.
  • Investigates problems, irregularities, policy violations, and other reportable events implementing the appropriate corrective action and follow-up.
  • Schedules and conducts initial nursing assessments for new patients.
  • Performs reassessments as required to determine the ongoing needs of patients.
  • Conducts Home Health Aide (HHA) supervisory visits in accordance with agency policies.
  • Ensures caregivers are providing care in accordance with patient’s care plan.
  • Investigates and follows up on incidents or clinical issues.
  • Collaborates with the office team to address and resolve concerns.
  • Completes all required nursing documentation associated with patient visits.
  • Submits medical orders, interim orders, incidents, accidents, and orientations, as necessary.
  • Develops, updates, and maintains comprehensive patient care plans.
  • Ensures care plans accurately reflect the patient’s ongoing needs.
  • Coordinates with the office team to ensure seamless communication and implementation of patient care plans.
  • Provides necessary information for scheduling and coordination of services.
  • Spends a significant amount of time visiting patient’s homes and medical centers.
  • Monitors the delivery of care and addresses concerns in real-time.
  • Promotes Agency to the community, participating in community programs with other professionals when necessary.
  • Provides orientation, in-service education, and additional training for Home Health Aides, Registered Nurses, and Professional Staff Personnel.
  • Ability to work on a part-time / per diem basis.
  • Other duties as assigned by supervisor.

Qualifications :

  • Current RN license in good standing.
  • BSN / MSN.
  • Minimum (2) two years of recent and verifiable clinical and managerial experience within a Home Care setting.
  • Cheerful personality and excellent communication skills.
  • Willing to meet and speak with patients, their families, and their medical professionals to develop, update, and maintain proper care plan.
  • Strong assessment and organizational skills.
  • Prior supervisory experience with field staff, and experience with clinical oversight, monitoring quality assurance, and overall case management.
  • Demonstrated successful track record of working independently.
  • Ambition to grow and advance beyond current position.
  • Solid communication and PC skills.
