Compensation : $140000 - $150000 Job ID : 29970
Job Function :
- Monday-Friday schedule, 9 : 30am-5 : 30 pm with NO WEEKENDS
- Providers provide routine primary care, wellness assessments and refer for specialty care
- Join a location with 5 other providers and a pharmacy onsite
- Provide patient care to patients of all ages (pediatric, adolescent, women's health and adults)
- Evaluate patients medication needs, write prescriptions
- Communicate with case workers, RN teams and other ancillary staff to ensure treatment plans are carried out
- Provide patient education around health promotion, disease prevention, and health maintenance
Job Requirements :
- NY State license, DEA, CSR (active or applied preferred)
- Training as a Nurse Practitioner or Physician Assistant
- 2+ years of primary care experience REQUIRED
