Job Summary : The Case Manager is responsible for coordinating the overall treatment experience for an assigned caseload.
Working as a part of a team with the Program Manager, Counselors, Recovery Support Specialists and Clinical Assistants, the Case Manager works individually with assigned clients developing and reviewing treatment, relapse prevention and continuing care plans to help the client develop a plan for achieving and maintaining long term recovery.
QUALIFICATIONS : Requires knowledge of person centered, strength-based counseling, case coordination, and treatment / recovery planning using ASAM Criteria.
Requires excellent communication, writing, and organizational skills and a working knowledge of Microsoft Office. Must have Addictions Counselor certification, a related professional license, or be eligible to obtain either within the required time frame.
Minimum of 12 months of continuous sobriety required for persons recovering from a substance use disorder. Submit to DHS Schedule L background checks and meet the eligibility requirements for employment. Bilingual preferred.
