Job Title : Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)
Location : Montour Falls, NY.
Shift Time : 3 : 00 PM to 11 : 00 PM.
Shift : Evening
Shifts per week : 5
Responsibilities :
- Administer medications, following physician's orders and nursing protocols.
- Monitor and report any changes in residents’ conditions to RNs or physicians.
- Provide direct patient care including wound care, IV management, and assistance with activities of daily living.
- Ensure accurate charting and documentation in the electronic health record, including using Point Click Care software.
- Assist in admissions, transfers, and discharges.
- Collaborate with the healthcare team to ensure the best possible outcomes for patients.
- Participate in care planning and assessments.
- Adhere to infection control standards and maintain a clean and safe environment.
- Assist with residents’ personal care as needed.
Qualifications : License :
License :
Active LVN or LPN license in the state of New York.
Experience :
Prefer previous experience in Skilled Nursing Facility (SNF) or Long-Term Care (LTC).
Certifications :
- Current Basic Life Support (BLS) certification.
