We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide essential nursing care and support to patients in various settings, including long-term care facilities, hospitals, and home health environments.
This role requires strong clinical skills, a commitment to patient-centered care, and the ability to work collaboratively with healthcare professionals.
Duties
- Conduct physical examinations and assess patient health status.
- Administer medications and monitor patient responses.
- Provide health coaching and education to patients and their families.
- Assist in diagnostic evaluations and sonography procedures as needed.
- Deliver hospice care to patients with terminal illnesses, ensuring comfort and dignity.
- Collaborate with registered nurses and physicians in developing patient care plans.
- Maintain accurate patient records and documentation in compliance with healthcare regulations
- Ensure a safe and clean environment for patients by adhering to infection control protocols.
Qualifications
- Valid LPN license in the state of practice.
- Proven experience in long-term care, hospice care, or medical-surgical nursing preferred.
- Strong skills in medication administration and patient assessment.
- Excellent communication skills for effective interaction with patients, families, and team members.
- Ability to perform diagnostic evaluations and assist in sonography when necessary.
- Compassionate approach towards patient care with a focus on health coaching.
- Experience working with children is a plus but not mandatory.
- Strong organizational skills and attention to detail.
