DescriptionEliot is offering an exciting opportunity to join a dedicated team of Mental Health professionals operating in the Lowell area.
The ideal individual will join us at our 9 bed Respite residential program and providing a high level of care through our Respite outreach service for adults with persistent & severe mental illness.
Our Respite Nurse candidate is comfortable working in a fast-paced setting and will have 2 years experience working with people with severe and persistent mental illnesses.
This person will provide oversight, guidance and assistance to clients in understanding their medical conditions; as well as performing any appropriate direct medical care that is needed.
Responsibilities : Provide education and consultation to consumers and staff regarding medical health, medication and medical topicsProvide 1 : 1 training in specific individualized need areasDevelop and conduct groups and trainings for consumers and staff in general areas of health and wellness including nutrition, exercise, smoking cessationAssist consumers with accessing community resources that will support and assist with addressing needs and health wellness effortsProvide oversight, guidance and assistance to consumers with understanding their medical conditionsMeet with consumers and staff to discuss any medical diagnoses and conditions, causes, symptoms, complications, and treatment optionsPerform direct medical care for a consumer as appropriate and ordered by physician which may include monitoring of vitals, wound care, etc.
This includes providing interventions specific to consumer IAP for medical / health and wellness goal areasReview all medical documentation for each consumer at respite and as assigned and provide medical care consultation and coordination as neededProvide assistance coordinating overall medical care for consumers and ensuring medical follow up is done as neededProvide interventions to consumers that focus on education and skills training in relation to health and wellness issues.
Assist the team and consumers by providing methods for on-going assessment of symptoms and treatment effectiveness and identify appropriate responses / actions.
