Position Highlights :

• FTE : Full and part time options available; share with us your availability
• Hours : 8 : 00am 4 : 30pm Monday Friday and 8 : 00am 4 : 30pm on weekends and holidays
• Work location : Home base of 1920 Libal St, Green Bay, WI 54301.
• Position travels to patient’s homes within the region (we offer mileage reimbursement and / or use of company vehicle)Serving patients in 9 counties in Northeast Wisconsin
• Salary : Based on experience and provided at time of interview
• Sign On Bonus available for qualified candidates
• Conduct in-home visits to complete a comprehensive physical assessment and provide care such as IV medication administration, IV site maintenance, wound care / wound vac application, and Foley / ostomy care.

Job Summary :

• Conduct in-home visits to complete a comprehensive physical assessment and provide care such as IV medication administration, IV site maintenance, wound care / wound vac application, and Foley / ostomy care.
• Nurses use advanced clinical skills to provide skilled, short-term acute care services, engage with one patient at a time, and build trusted patient relationships.
• Provide patient education and coaching to make a meaningful impact and improve patient outcomes.
• Work collaboratively with the multi-disciplinary health care team and community resources to provide the best outcomes for patients.

Qualifications :

• Wisconsin LPN or RN license required.
• Valid Wisconsin driver’s license in good standing and liability car insurance required.
