About The Position
Job Title : Supervisor - Case Management Job Specialty : Case Management Job Duration : 6 Months Shift : Day - 5x8 - 8 : 00 AM - 4 : 30 PM, 40 Hours Guaranteed Hours : 40 Experience : Minimum 2 years of current acute care case management leadership experience, inpatient hospital case management experience, discharge planning, and utilization management experience.
License : Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license. Certifications : Certification in Case Management (CCM) from the Commission for Case Management Certification (CCMC) preferred.
Must-Have : Epic and Careport experience- Acute care case management experience in a hospital setting- Recent experience during and following the height of Covid- Leadership experience in a lead or manager capacity- Experience in counseling new hires on difficult conversations with MDs and families Job Description : Responsible for overseeing daily case management operations.
- Monitor and guide a team of professionals in collaborative practices including assessment, planning, facilitation, care coordination, evaluation, and advocacy.
- Ensure patient safety, quality of care, and cost-effective outcomes through effective communication and resources.- Participate in developing policies, procedures, workflow design, and performance improvement activities.
- Evaluate competency-based performance of staff members and ensure the effectiveness of care interventions.- Provide constructive feedback and collaborate with case managers for professional development goals.
- Serve as a resource for case managers handling complex social, emotional, and medical needs.- Collaborate with management to optimize case management best practices and develop educational content.
- Provide leadership for performance improvement.- Lead workflow enhancement activities and organize orientation and continuing education for staff.
- Mentor and manage personnel, focusing on developing the skills of new hires.Candidates are required to work within the designated metro area and should not have hospital experience solely in inpatient psychiatry.
Candidates should have a background in inpatient settings of various hospital sizes.
