Job Description :
- Patient Care : Deliver comprehensive home health care to patients, including assessments, wound care, medication administration, and personalized care plans.
- Health Monitoring : Regularly monitor and document patients' health status, vital signs, and progress to ensure optimal care and timely intervention.
- Care Coordination : Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
- Patient Education : Provide education and support to patients and their families regarding care techniques, health maintenance, and disease management.
- Home Environment : Ensure a safe and conducive home environment for patients, advising on necessary adjustments and safety measures.
- Documentation : Maintain accurate and detailed patient records, ensuring compliance with all regulatory requirements.
Education + Certification Requirements :
- Current Registered Nurse (RN) license in the state of Nebraska.
- Bachelor of Science in Nursing (BSN) preferred.
- Current CPR certification.
- Strong organizational and communication skills.
- Ability to work independently and manage time effectively.
