- Familiarity with community health practices and integrated care models preferred. Responsibilities :
- Conduct comprehensive assessments to identify mental health needs.
- Develop, implement, and evaluate individualized treatment plans.
- Provide therapy and counseling to individuals, families, and groups.
- Collaborate with multidisciplinary teams to ensure integrated care.
- Maintain accurate and up-to-date patient records in the electronic health record system.
- Facilitate case management and connect clients with community resources.
- Provide crisis intervention and support as needed.
- Participate in training and supervision of interns or support staff.
