Custom Home Health is seeking a dedicated SOC Registered Nurse to join our talented team on a full-time basis, supporting patients in Oakland County.

Position Responsibilities :
- Excellent communication skills to effectively interact with patients and interdisciplinary team members.
- Supportive team member who understands the importance of collaboration and teamwork.
- Ability to meet the physical demands of providing nursing care, including ambulating patients, transferring patients, and assisting with exercises.
- Assume responsibility for an assigned caseload of patients, conducting OASIS assessments and making planned home visits according to the Plan of Care.
- Coordinate with therapists, home health aides, and other team members to ensure consistent and appropriate care for each patient.
- Share responsibility for on-call duty for patients in emergency situations and on weekends, including home visits as required.
Requirements :
- Graduate of an accredited school of nursing; BA degree preferred.
- Minimum of one year of clinical experience, with home care experience required.
- Kinnser training preferred; Oasis experience required.
- Current license in good standing as a Registered Nurse in the State of Michigan.
- CPR certification required.
- Current Michigan driver’s license and / or reliable transportation with proof of automobile insurance of personal vehicle.
