Penn Medicine is seeking a Nurse Practitioner to join the Heart Transplant unit at CPUP and HUP. This position is outpatient with inpatient rotations.
The Penn Transplant Institute has been a pioneer in organ transplantation for nearly fifty years. Penn's multidisciplinary team leads the region in the total number of transplants performed and is ranked among the top 10 multi-organ transplant centers in the country.
The Penn Medicine Heart Transplant Program was established in 1988 and has performed over 1,500 life-saving heart transplants.
Penn is one of the largest heart transplant programs in the country and offers exceptional, innovative and personalized longitudinal care for patients with end-stage heart failure and those in need of multi-organ transplantation.
Position Description :
The heart transplant advanced practice provider (APP) provides and maximizes the quality of care to patients referred to Penn with end-stage organ failure.
The heart transplant APP provides direction in the management of patient care throughout all phases of the transplant process : organ failure, transplant evaluation, waitlist management, post-operative and longitudinal follow-up care.
Multidisciplinary care delivery occurs in the ambulatory and inpatient settings using a combined approach of in-person and telemedicine monitoring.
The heart transplant multidisciplinary team includes heart failure / heart transplant physicians, cardiac surgeons, heart transplant APP and RN's and transplant social work, pharmacists, nutrition, financial and patient service coordinators.
The multidisciplinary team includes integrated care coordination with a variety of specialized consultants including transplant infectious disease, transplant psychiatry, endocrinology, hepatology, nephrology, pulmonology, histocompatibility, cardiac pathology, cardiac rehabilitation, physical therapy, etc.
The heart transplant APP serves as an integral member of the transplant team in meeting the needs of the transplant patient and ensuring continuity of care throughout the transplant process.
The heart transplant APP performs a systematic assessment, evaluation and develops a plan of care for patients and their significant others;
develops a written, continuous record for each patient; implements identified medical and nursing interventions; and evaluates outcomes.
This is an excellent opportunity for an APP to provide longitudinal care to a complex population and be a well-respected member of the team.
Minimum Requirements :
Licensed to practice in the Commonwealth of Pennsylvania, M.S.N., and certification as a Nurse Practitioner. Acute Care certification required
Prior to employment, the AP must be reviewed by the Credentials Committee and approved by the Medical Board
Additional Information :
This position is Monday through Friday 40 hours per week and requires rotational inpatient weekend coverage, along with shared on-call duties for after-hours, weekends and holidays.
Hours may be adjusted to meet the operational needs of the department.
