As a Physical Therapist, you will be responsible for evaluating and treating patients under physician orders using various therapeutic techniques.
Your goal will be to restore muscle strength, coordination, body alignment, relieve pain, and improve movement and function.
You will work as a part of the interdisciplinary team, providing valuable support and information within the realm of Physical Therapy.
Responsibilities :
1. Analyze resident information and incorporate it into the assessment and treatment process.
2. Perform a comprehensive functional assessment of the resident, including evaluation of home / vocational needs and equipment requirements.
3. Develop individualized treatment plans with achievable functional goals based on the resident's specific needs.
4. Adhere to evaluation, treatment, and documentation guidelines, ensuring accurate and timely recordkeeping.
5. Train and supervise Assistants during treatment procedures to ensure quality care.
6. Document the progress of treatment in resident records after each session.
7. Continuously revise treatment plans as necessary throughout the treatment process.
8. Provide clear and concise discharge summaries in a prescribed format.
9. Orient residents to the program services and potential risks associated with procedures.
10. Collaborate with nursing staff, residents, and families to maximize functional abilities.
11. Educate residents, families, and caregivers on exercise programs and therapeutic procedures for continuation at home.
12. Assist in the referral process for residents requiring additional services after discharge, while staying informed of community resources.
13. Attend and actively participate in rehabilitation team conferences to discuss resident progress, problems, and needs.
14. Attend other department and facility meetings as required.
15. Pursue continuing education opportunities to maintain clinical competency.
16. Contribute to the quality improvement process.
17. Promote awareness of Heritage Healthcare programs and services through interactions with the community.
18. Adhere to professional standards, guidelines, and regulatory requirements set by Heritage Healthcare.
19. Provide input on the clinical performance of assistants as requested by the Rehab Manager.
20. Comply with facility policies and participate in quality improvement and safety programs.
21. Perform additional duties as assigned to ensure quality care for residents.
22. Understand reimbursement protocols of various payers and allocate therapy resources accordingly to achieve discharge goals.
23. Demonstrate knowledge of OBRA regulations, HIPAA, Local Coverage Determinations, and third-party reimbursement guidelines.
24. Support the company's commitment to Compassionate Care and maintain a professional image to instill confidence in the Rehab Program.
25. Work independently and recognize situations where direct supervision is needed, while actively assisting others.
26. Maintain expected productivity levels as discussed and expected for your specific facility / facilities.

Qualifications :
1. Hold a Bachelor's or Master's degree in Physical Therapy.
2. Possess a current license to practice in the state.
3. Preferably have experience in a setting serving a similar age / type of resident population.
4. Obtain applicable certifications and demonstrate competency in specialized areas.
5. Excellent oral and written communication skills.
6. Proficient in the principles, methods, materials, and equipment used in physical therapy.
7. Able to assess resident needs and create comprehensive care plans.
8. Familiarity with accreditation standards and compliance requirements.
9. Capable of performing physical activities such as standing, pushing, lifting, stooping, reaching, and balancing in a safe manner.
