About the Role :

We are seeking a licensed Physical Therapist to join our client's Home Health team. You'll provide in-home Physical Therapy services, assess patient functioning, and develop individualized treatment programs to improve independence and physical functioning.

This role includes patient and caregiver education, supervision of assistants, and reporting on patient progress.

Your Mission :

• Assess & Treat : Perform patient assessments and develop comprehensive treatment programs.
• Monitor & Document : Record patient responses to treatment and any condition changes.
• Educate & Support : Instruct patients and caregivers in therapeutic programs and assistive device use.
• Supervise Team Members : Oversee physical therapy assistants and, when applicable, certified nursing assistants.
• Collaborate & Communicate : Prepare reports and engage in staff and agency development activities.

Key Responsibilities :

Schedule : Monday-Friday, 8 : 00 AM / 9 : 00 AM - 5 : 00 PM + 1 weekend per month (day shift with scheduled patients);

2 holidays per year (1 summer, 1 winter); no on-call.

• Travel : Local travel to residential homes within Chicago and surrounding areas.
• Caseload Requirement : Full-time, 40 hours / week with a goal of 30 points per week.

Requirements :

• Education : Master's degree or higher in Physical Therapy.
• Licensure : Valid Physical Therapist license in Illinois (IL).
• Experience : 1+ year of experience in a healthcare setting, with 1+ year in home health or inpatient rehab required.
• Skills : Proficiency in OASIS and HCHB preferred; however, training is provided.
• Driver's License : Valid regular driver's license (DL).

• Pay Structure : PPV rates (see attachment for details) + $10,000 first-year service bonus.
