LSSI is hiring a full-time Assistant Director of Clinical Services . Assist the Director of Clinical Service in completing the necessary assessments and related paperwork and communications to maintain supportive living residents and provide supervision of Supportive Living Staff including CNA’s. Essential Functions :
- Assist Director of Clinical Services in completing initial, quarterly and annual resident assessments; and all other required service plans and assessments.
- Assist Director of Clinical Services in scheduling staff, maintaining / assist with staff coverage, and supervising Supportive Living Staff.
- Follow all standards regarding safety, licensing, regulatory and agency requirements.
- Responsible for maintaining resident files in accordance with HFS requirements and completing resident care plans.
- Assist with medication set up and dispensing; communicate with pharmacy regarding resident medication and concerns.
- Assist with providing personal care services for residents, including but not limited to assistance with bathing, eating, dressing, personal hygiene, grooming, toileting, ambulation and transfer.
- Provide direct supervision, training, support, review, and evaluation for assigned staff.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizationsand / or individual in a dignified, respectful, honest and fair manner.
Position Qualifications :
- High school diploma or equivalent required.
- Must be at least 18 years of age.
- Background check clearance, including Health Care Worker Background Check, required.
- Licensed Practical Nurse (LPN) by the State of Illinois or ability to obtain LPN licensure within one year of date of hire.
- Certified in emergency resuscitation.
- Minimum of two (2) years’ experience in health care services to adults with disabilities or the elderly population either in assisted living, inpatient hospital, long-term care setting, adult day care, or in a department approved health related field.
- Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
- Willingness to participate in required trainings to meet requirements of the SLF program.
- Excellent organization, presentation and pc / computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
- Valid driver’s license, in good standing for the state of residency required.
- Access to reliable transportation required.
- Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
