The Licensed Practical Nurse (LPN) provides healthcare services to patients of Primary and Specialty Care Services. They collaborate with and support team members to ensure a patient focused approach to care.
As part of the essential functions of this role, the Licensed Practical Nurse :
- Assesses, plans, implements and evaluates care for patients under their scope of practice; Prepares patients for physician visits by taking vital signs, history and assessing risk factors
- Collects and records health information findings of physical and developmental assessments into patient record with timeliness and accuracy.
Accesses and retrieves medical records via HEALTHeLINK
Assists and accepts all telephone calls from patients and directs them to appropriate avenue for treatment; Screens all new and returning patients with consent forms for completing the intake process to include obtaining all previous medical records and documentation;
Documents intake encounters and health history in the EMR (Medent)
- Performs all diagnostic tests deemed appropriate by physician or nurse practitioner including EKGs, Nebulizer treatments and other clinical procedure assistance as needed
- Assists and performs all clinical and related administrative functions for the laboratory and HIV / STI / Hepatitis screening program as assigned
- Assists and prepares all paperwork for the Medical Director and / or Director of Nursing, approval(s) surrounding prescriptions, patient benefits, entitlements, disability, other medical provider and agency inquiries
- Performs phlebotomy for testing and administers immunizations and vaccinations as ordered by licensed provider including all written or standing orders
- Assists physicians, nurse practitioners, and registered nurses with all procedures (pap smears, anoscopy, ear lavage etc.
and the processing of all related specimens as needed
Qualified Candidate will have current LPN license and current BCLS certification with a minimum of one (1) year of experience working in a medical practice setting, preferably in an HIV / AIDS or other infectious disease practice.
Prior phlebotomy experience required. Candidate must be able to communicate clearly and professionally in writing and verbally.
Candidate must demonstrate familiarity with computers and MS Office software (Outlook, Word) and electronic charting / medical records systems (Medent preferred).
Candidate must demonstrate personal responsibility and integrity and ability to handle Protected Health Information (PHI) in a manner consistent with HIPAA.
Sensitivity to and experience with cultural diversity, Harm Reduction, LGBTQ population, Substance Use population and people living with HIV / AIDS is essential.
