Yale New Haven Health, the largest and most comprehensive healthcare system in Connecticut, is inviting Physicians to join an established Hospitalist Service at Yale New Haven Hospital.
This position will provide care virtually to patients at home in our Home Hospital program. Day shifts are 12 hours duration from an on-site location.
Some shifts may include rounding on potential Home Hospital patients at Yale-New Haven or Bridgeport Hospital depending on the need.

Responsibilities
- Provide direct and consulting care to inpatients in the Home Hospital units (which span multiple hospitals).
- Work in a team environment (Physicians, NPs, RNs) in Mission Control and communicate with individuals in the home.
- Integrated care with primary care providers, specialists and ancillary departments.
- Ensure compliance with regulatory agency requirements (e.g. JCAHO).
- Review clinical staff performance to ensure quality of care and efficient use of resources.
- Maintain clinical activity by full participation in departmental clinical workload.
- Participate in medical staff leadership activities
- Prepares for, attends and participates in team meetings and daily huddles
- Collaborates in developing and attaining team priorities, patient goals, and care plans
- Keeps problem list, medication list and patient care plan updated for team members
- Approves orders and referrals for health maintenance
Qualifications :
Qualifications / Education :
Medical degree (MD, MBBS, DO) and Internal or Family Medicine Residency completion with a total of at least 4 years of clinical working experience as a physician within the inpatient settings or combination of inpatient and outpatient settings post residency.
Any of the following working experiences count : Internal Medicine, Family Medicine, Geriatrics and Emergency Medicine.
Licensure / Certification :
Connecticut State Licensed
Board certified
ACLS / BLS Certified
DEA License
Experience :
Completion of the above educational programs.
Special Skills :
Demonstrated aptitude and interest in high quality, coordinated care delivered virtually.
Physical Demand :
MMR and influenza vaccine is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood / body fluids.
Tdap vaccination are strongly encouraged. Standing, walking and sitting most of day. Working with lightweight equipment.
