Job Title : Nurse Practitioner - Primary Care

We are seeking a passionate Nurse Practitioner to join us in delivering outstanding primary care services to our patients in Jacksonville, FL at clinics in Arlington, Brentwood, Normandy, Jax Heights & Orange Park.
Responsibilities :
- Conduct comprehensive primary care assessments, diagnosis, and treatment of patients
- Develop and implement patient care plans in collaboration with the interdisciplinary team
- Build strong, lasting relationships with patients to provide personalized care and support
- Educate patients on health maintenance and disease prevention strategies
- Utilize cloud-based electronic health record system for documentation and communication
Qualifications :
- Board Certified Nurse Practitioner (NP, APRN, AGNP, or CNP)
- Minimum of 2 years of experience in a primary care outpatient setting
- Geriatric experience preferred
- Bilingual Spanish speaking skills a plus (not required)
- Passionate about providing high-quality care and contributing to a fast-growing organization
