Responsibilities :
- Develop and implement nursing policies and procedures for home health services to ensure compliance with healthcare regulations and standards.
- Oversee and manage the clinical team, including nurses and support staff, to provide high-quality patient care and support.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care plans and ensure effective communication within the healthcare team.
- Monitor and evaluate patient outcomes and satisfaction levels to identify areas for improvement and implement strategies to enhance the quality of care provided.
- Provide leadership and mentorship to nursing staff, fostering a positive work environment and promoting professional development and growth among team members.
Additionally,
- Consult, plan, and work with the Administrator in policy formulation and programmatic development and implementation including assessing adequate and appropriate staff.
- Evaluates the program, services, and field personnel.
- Directs the implementation of service goals and objectives.
- Develops standards that ensure safe and effective services to clients and families.
- Establishes job functions, qualifications, and job descriptions for each service staff (direct and supervisory).
- Keeps program personnel up to date with state-of-the-art clinical information and practices staff development including orientation, in-service education, and continuing education.
- Maintains policy and procedure manuals and updates procedures and job descriptions as policies and procedures change.
- Directs staff in the performance of their duties.
- Oversees the implementation and evaluation of client care services inclusive of regulatory licensure and certification criteria and accreditation standards.
- Assesses appropriate staff supervision during all service hours.
- Assists in the evaluation of community needs and plans programs and services to meet identified needs.
- Assists in agency evaluation activities.
- Assists in marketing the agency and fostering community relations.
- Makes patient and personnel assignments.
- Coordinates patient care.
- Coordinates referrals.
- Ensures the development, implementation, and updates of the individualized plan of care.
Qualifications :
- Active RN license in the state of Virginia.
- MUST HAVE : 2+ years of experience in home health.
- MUST HAVE : 1+ years of supervisory experience in home health.
- Must be able to pass a criminal history background check.
- A Bachelor’s degree is preferred.
