Responsibilities :
- Perform clinical assessment and identify patient needs.
- Document, prepare, and complete patient Plan of Care. Track progress and revise as needed.
- Meet mandatory continuing education requirements.
- Plan and provide appropriate treatment for neuro-muscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
- Assist and instruct patients and families in transfer techniques appropriate to patient abilities. Instruct patients and families on the use of equipment, such as walkers, wheelchairs, canes, etc.
in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs.
Promote company mission and administrative policies to ensure the quality of care.
Requirements
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association-approved program
- Current Texas State License as a Physical Therapist
- Current Texas Driver's License and reliable transportation
- Six months of experience as a Physical Therapist in an acute care or rehabilitation setting. Pediatric experience is preferred.
- Effective interpersonal relations and communication skills
- Knowledge of durable medical equipment
- Ability to work a flexible schedule and travel locally
- Able to work in a patient's home in various conditions
- Excellent organizational and time management skills
- Proficient in Microsoft Office
- Able to maintain a high level of confidentiality
- Proof of current CPR certification
- Ability to do heavy lifting, bending, pulling, pushing, and standing. Prolonged standing and walking are required.
- Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient / family individualized psychosocial needs.
