Brigham and Women's Faulkner Hospital’s Home Hospital is an innovative clinical service that helps our patients maximize their time at home.
When illness develops and is significant enough to require a hospital stay, Home Hospital can deliver hospital-level care in the comfort of the patient’s home!
Common conditions treated by Home Hospital include heart failure, COPD, pneumonia, skin infections such as cellulitis, urinary tract infections, and as well as conditions if deemed safe to do so. Services include :
- Vital sign monitoring (heart rate, blood pressure)
- Lab testing
- Intravenous (IV) fluids and medications
- Oxygen
- Continuous heart monitoring
- X-rays
- Ultrasounds
The Opportunity
We are seeking a per diem Administrative Coordinator to support Home Hospital onsite at Brigham and Women's Faulkner Hospital and cover open shifts due to gaps in staffing, vacations, call outs, etc.
Role Overview
Under the direction of the Administrative Manager, the Administrative Coordinator is an integral member of our multi-disciplinary team.
Not only do they work in close collaboration with the nurses, providers, and managers, but they also serve as the liaison between various internal groups / departments / external resources and work directly with patients and families to coordinate screening, consent, and enrollment into the Home Hospital Service.
Administrative support responsibilities include tracking referrals, admissions, document management, communicating all aspects of the workflow status, promoting an ongoing culture of collaboration and efficiency throughout the patient intake process, scheduling, collection of patient supplies and equipment, and other tasks / projects as assigned.
This is a unique opportunity to work autonomously supporting the BWFH’s Home Hospital Service, while also functioning as part of the newer, larger Mass General Brigham Home Hospital team supporting entities across our system.
As foundational hires, our Administrative Coordinators will be helping us build this function out and define what this model will look like future-state!
