Assertive Community Treatment (ACT) is a comprehensive community-based treatment program for persons with severe and persistent mental illness.
The ACT Team is a multi-disciplinary group of mental health professionals organized to help provide treatment, rehabilitation, and support services to consumers in the community.
Required Competencies
We are offering a stay Bonus of $1,000.00 to be paid $250.00 at 3 months, $250.00 at six months and $500.00 at a year.
Hourly : $18.00+ depending on experience
Job responsibilities
- Provide services in the community and homes as appropriate
- Flexibility to adjust service to meet an individuals needs
- Coordinate and implement housing services and resource based on individual need
- Develop and maintain relationships with local housing vendors
- Manage and facilitate referrals to housing providers
- Assist with creating new hosing opportunities
- Assist with completing applications and others housing related paperwork, such as housing vouchers,
- Work on independent living skills
- Education regarding financial responsibilities and budgeting
- Working with Rep Payees
- Treatment planning, assessments and on going paperwork
- Being liaison between landlord and individual to discuss / resolve concerns
- Assist in providing ongoing assessment of individuals mental health and symptom management
- Provide case management
- Crisis intervention services and on call
Ability to Access Community Resources - The Housing Specialist is responsible for determining the eligibility of individuals on housing opportunities, coordinate and implement housing needs / services, assist individuals in the procurement of safe and affordable housing and related necessities, and develop and maintain relationships with local housing vendors.
Flexibility - Provide 24 hour a day, 7 day per week on-call crisis services on a rotational basis.
Time Management Skills- Maintain case records and complete appropriate documentation, letters, assessments and summaries as needed or designated by established policies and procedures.
Meeting Basic Work Expectations -Complies with basic job requirements and tasks; arrives at work on time as scheduled; proactively informs others when he / she encounters problems that may limit his / her ability to meet expectations.
Essential Job- Related Responsibilities
Service Plan Development - Develops treatment plans and crisis plans based on the client’s strengths and needs. Gains an understanding of the client’s relevant personal, familial, social, vocational, and cultural data.
Adherence to charting requirements- Prepares and completes all necessary documentation, including assessments, new consumer admissions, medication records, progress notes, incident reports, treatment and crisis plans and miscellaneous paperwork / reports within required timeframes as requested.
Benefit Acquisition and Maintenance - Assists the client in gaining funding authorization for needed services and entitlements.
Performs other related duties as assigned- Develops and maintains relationships with local housing vendors, which include private landlords, the local housing authorities, personal care homes and other providers which offer housing to the clients.
Manage and facilitate referrals to housing, Responsible for creating new housing opportunities within community and maintaining contact with landlords and residential providers, Meet with potential vendors to discuss the client’s needs, Coordinate and support arrangements for lease agreements, security deposits, financial subsidies and / or rental and homeowner’s insurance, Complete any paperwork necessary for an individual to receive a housing voucher, be placed on a housing list, or receive housing assistance, Keep abreast of any local and state housing initiative, changes and opportunities for the clients, Provide client education for household safety issues, Work with payee with regards to housing expenses and be the liaison between the individual and their landlord to discuss / resolve any issues / concerns.

- Drug testing
- Education verification and other credentialing based on position requirements.
- Proof of employment history or references (if required)
