ABOUT THE ROLE

  • The Registered Nurse Case Manger (RN Case Manager) will provide direct patient care services to hospice patients and supportive care to the patient’s family;
  • manage an interdisciplinary group (IDG) consisting of Licensed Practical / Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care;

and case manages all assigned patient clinical records.

This schedule will be Monday- Friday 8 : 00am-5 : 00pm*

ROLE REQUIREMENTS

  • Registered Nurse of an accredited school of nursing with a current license to practice professional nursing in the state in which we do business.
  • One (1) year of experience in oncology, long term care, hospice or home health setting.
  • Current Cardiopulmonary resuscitation (CPR) certification.
  • Must have reliable transportation, proof of current automobile insurance and a valid driver’s license.
  • This candidate must be willing to travel*
  • This candidate must have 1 year of hospice experience*
  • This will be serving the areas of North Phoenix / Scottsdale *
