Position Description :
Under the general direction of the UM Manager, this RN professional will play a major role in the collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for Medicare / Medicaid options and services to meet the comprehensive health needs of each patient and / or family.
Utilizing effective communication skills and available resources, these RNs will promote quality, cost-effective outcomes, including care coordination for complex conditions, and support coordination of care efforts.
As a complex Cal Mediconnect Case Manager you will carry an approximate caseload of 130-140 patients, all of whom would remain active 100% of the time.
You must have great communication skills for engaging members.
