Planet Healthcare is seeking a CNA Hospice for a local contract job in Sellersville, Pennsylvania.

CNA - Hospice Aide - Sellersville, PA n8 weeks, Days nShift : 8a-4 : 30p, 40hrs / wk nStart : 10 / 21 n nLocal only : $25 / HR n nMust be a Certified Nurse Aide.

Must have home health or hospice patient experience. Patient population seen is a mixture of home bound and patients within a facility.

Average of 6 patients per day and approximately 1 hour visits per patient. Must reside within 25 miles of Grand View Health.

Mileage is reimbursed for patient to patient visits. Shifts can be either 8am-4 : 30pm OR 7am-3 : 30pm. 2 monthly meetings are held in the Hospice office onsite of the main facility.

n nThe Home Hospice Aide provides and assists with personal care to patients at their home according to established standards in order to maximize patient comfort.

n nAssesses the patient's temperature, pulse, respirations and blood pressure to monitor his / her condition. n nAssists patients with prescribed exercises that the patient and Hospice Aide have had instruction by Registered Nurse or Physical Therapist to facilitate patient benefit from exercise program.

n nAssists patient with application of TED / JOBST stockings. n nObserves patient's condition and reports changes in condition to the Registered Nurse or Supervisor.

n nAssists with patient transfers, ambulation and prescribed exercises according to established procedures. n nDocuments care provided and other pertinent observations in the clinical record to promote continuity of care.

n nCompletes patient assignment within acceptable time frame and meets productivity standards. n nEffectively cares for the dying patient / patient's family.

n nAssists with light housekeeping tasks as indicated by HHA Plan of Care. n nGrand View Health's Hospice department serves Bucks and Montgomery communities.

The Hospice staff is expected to commute within a 30-minute radius of Grand View Hospital. n nEDUCATION AND EXPERIENCE : n nHigh school diploma / GED required n n nQUALIFICATIONS : n nBasic Life Support (BLS) n nValid Pennsylvania driver's license and auto insurance required.

n nCertified Nursing Assistant (CNA).
